Location: 54 Holly Tree Lane, Jamesport, NY 11947. On or about 03/28/2025, Employees were not wearing protective helmets while working directly below overhead plywood installation and were exposed to being struck by materials and tools. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

Employees were not protected from falling, while installing plywood sheathing on the roof of a residential home approximately 25ft above the ground. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. Location: 54 Holly Tree Lane, Jamesport, NY 11947. On or about 03/28/2025, Employees were using a 32-foot extension ladders set up on the front of the building to gain access to the roof of a two story residential home to install plywood sheathing. The ladder did not extend three feet above the landing. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
