29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) School Roof: On or about April 4, 2025, an employee was exposed to fall hazards while working on a roof approximately 11'9" above the lower level, without any fall protection.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user.  a) Jobsite: On or about April 4, 2025, the employer failed to create and implement a written respiratory program that ensured an employee was medically able to wear a full face respirator during scraping and caulking operations.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) Jobsite: On or about April 4, 2025,  the employer did not create a written certification record verifying that employees received adequate fall protection training before being exposed to fall hazards.
