On or about April 2, 2025, and times prior to, Bellerive Country Club did not ensure that employees had the required training and evaluation completed prior to operating a Class V, Toyota Tailift PFG25, powered industrial vehicle. Employees were exposed to struck-by hazards when moving pallets of materials at a worksite located at 12925 Ladue Road in Saint Louis, Missouri.

29 CFR 1910.243(e)(1)(i): Power lawnmowers of the walk- behind, riding-rotary, and reel power lawnmowers were not guarded in accordance with the machine guarding requirements in 29 CFR 1910.212, General requirements for all machines: On or about April 2, 2025, and times prior to, employees of Bellerive Country Club, located at 12925 Ladue Road in Saint Louis, Missouri were utilizing John Deere Walk Greens 180 E-Cut reel type mowers on golf course greens. The employer did not ensure the operator's presence controls were functioning properly exposing employees to amputation hazards when emptying the grass catcher bins located near the point of operation.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in an in-patient hospitalization, amputation, or the loss of an eye: On or about March 24, 2025, at approximately 10:00 a.m. an employee sustained a three-finger amputation and was transported to the local hospital. The employer did not report the injury to the Occupational Safety and Health Administration (OSHA) within the required twenty-four (24) hours. OSHA was notified on March 27, 2025, at approximately 4:00 p.m.
