29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). Location: Jobsite at 52 School St., Everett, MA On or about April 4, 2025, workers were permitted to work inside an unprotected excavation that was greater than five (5) feet deep. They faced potential cave-ins from the unsupported walls of the excavation and from the spoil pile, which was not set back at least 2 feet from the edge of the excavation.
