The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a. Located at 699 Wellingshire Blvd, Columbus, OH 43085: On or about April 4, 2025, the employer did not provide a fall protection training program to ensure that employees were protected from fall hazards while working at heights of approximately 13 feet. The employees were performing re-roofing activities on the clubhouse of an existent residential complex.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a. Located at 699 Wellingshire Blvd, Columbus, OH 43085: On or about April 4, 2025, the employer failed to ensure employees were protected against fall hazards of approximately 13 feet. The employees were performing re-roofing activities on the clubhouse of an existent residential complex. Isidoro Segura Tapia was previously cited for a violation its equivalent Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1776838, citation number 1, item number 1, and was affirmed as a final order on December 19, 2024, with respect to a workplace located at 5991 - 6005 Eden Valley Drive, Westerville, OH 43081.
