29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS) On or about 4/7/2025, the employer did not ensure portable ladders were constructed and placed so three (3) feet or more were extended above the upper landing at the job site located at the 2764 Spring Water Ct., in Oakville MO. The ladder the employees used to access the roof was extended approximately half a foot beyond the upper landing, or edge of the roof, thus exposing employees to fall hazards.
