29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) On or about 4/7/2025, ARS Construction did not conduct an adequate inspection of the job site, materials, and equipment. Employees were exposed to the following hazard at the work site located at 2758 Spring Water Ct., in Oakville MO: a) The roofing subcontractors were replacing a roof on a residential structure greater than ten (10) feet above ground. The crew was exposed to fall hazards due to not properly utilizing a personal fall arrest systems (PFAS) and by using a ladder that was not three (3) above the upper landing surface. b) The general contractor controlling the work was onsite monitoring the work during the time of the inspection.
