29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a)AL General Construction at 910 Prairie Park Drive, Beresford, SD 57004:  On or about and at times prior to April 3, 2025, the employer did not ensure that employees were protected from falls while conducting roofing activity on a single-family home.  This condition exposed employees to fall hazards between 14 and 19 feet.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)AL General Construction at 910 Prairie Park Drive, Beresford, SD 57004:  On or about and at times prior to April 3, 2025, the employer did not ensure employees engaged in roofing activities were trained to recognize the hazards of working at elevation and in the use of fall protection systems. Employees were working adjacent to the edge of the roof without the use of a fall protection system.  This condition exposed employees to fall hazards between 14 and 19 feet.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)AL General Construction at 910 Prairie Park Drive, Beresford, SD 57004:  On or about and at times prior to April 3, 2025, the employer did not ensure that the Mighty Multi Extension Ladder being used to access the roof of a single-family home was at least 3 feet above the upper landing surface.  This condition exposed employees to fall hazards greater than 14 feet.

29 CFR  1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  (a)AL General Construction at 910 Prairie Park Drive, Beresford, SD 57004:  On or about and at times prior to April 3, 2025, the employer did not ensure employees were provided a training program for each employee utilizing the Mighty Multi Ladder for the procedures to be followed to minimize hazards related to the use of ladders for access to the roof of a single-family home to conduct roofing activities.  This condition exposed employees to fall hazards between 14 and 19 feet.  Abatement Note:  The employer shall ensure that each employee has been trained by a competent person in the following areas, as applicable:  (a)	The nature of fall hazards in the work area;  (b)	The correct procedures for erecting, maintaining, and disassembling the fall protection systems to be used;  (c)	The proper construction, use, placement, and care in handling of all stairways and ladders;  (d)	The maximum intended load-carrying capacities of ladders; and  (e)	The standards contained in this subpart.
