29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. On or about April 2, 2025, at the facility, employees were replacing the extension motor on the FeedMAX Block Push Bottom Feeder, during which time the energy control procedure did not clearly and specifically outline the techniques to be utilized for the control of gravitational energy; therefore, exposing the employees to amputation and caught in between hazards.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: a) On or about April 2, 2025, at the facility, a maintenance employee replacing a motor on the FeedMAX Block Push Bottom Feeder had not received training on the employer's energy control program, exposing them to gravitational and mechanical energy during servicing.
