29 CFR 1926.251(a)(6): Each day before being used, and during use, where service conditions warrant, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person, and damaged or defective slings were not immediately removed from service: On or about April 1, 2025, at the cement terminal under construction, a competent person did not inspect the sling and all fastenings and attachments for damage or defects and damaged or defective slings were not immediately removed from service while employees were waiting to install a 40,000-pound truss from a JLG 1250 AJP Boom lift.

29 CFR 1926.251(e)(8)(iii): Synthetic web slings were not immediately removed from service when any of the following conditions were present: snags, punctures, tears or cuts. On or about April 1, 2025, at the new concrete structure jobsite, employees used synthetic slings that showed snags, punctures, tears and cuts while lifting a metal truss with a crane. The damaged slings were not removed from service.
