29 CFR  1910.147(c)(1):Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a) On or about 04/08/2025, the employer failed to establish and implement an energy-control program which provided training, performed periodic inspections on procedures, and developed procedures, documented, and utilized the control of potentially hazardous energy associated with the Slitter Rewinder Rhyguan which the employees perform  roll changes, blades changes, and clean the machine without performing LOTO. The lack of machine-specific energy control procedures exposed employees to hazardous energy during servicing operations.  b) On or about 04/08/2025, the employer failed to establish and implement an energy-control program which provided training, performed periodic inspections on procedures, and developed procedures, documented, and utilized the control of potentially hazardous energy associated with the Slitter Rewinder Vertaflex Racer  which the employees perform roll changes, blades changes, and clean the machine without performing LOTO.were performing [insert specific servicing/maintenance task]. The lack of machine-specific energy control procedures exposed employees to hazardous energy during servicing operations.  c) On or about 04/08/2025, the employer failed to establish and implement an energy-control program which provided training, performed periodic inspections on procedures, and developed procedures, documented, and utilized the control of potentially hazardous energy associated with the Comco Press 7 inch 2 color Cadet which the employees perform die changes, roll changes, blades changes, change the ink and clean the machine without performing LOTO.]. The lack of machine-specific energy control procedures exposed employees to hazardous energy during servicing operations.  f) On or about 04/08/2025, the employer failed to establish and implement an energy-control program which provided training, performed periodic inspections on procedures, and developed procedures, documented, and utilized the control of potentially hazardous energy associated with the Comco 10 inch 10 color Cadet which the employees perform die changes, roll changes, blades changes, change the ink and clean the machine without performing LOTO. The lack of machine-specific energy control procedures exposed employees to hazardous energy during servicing operations.  g) On or about 04/08/2025,the employer failed to establish and implement an energy-control program which provided training, performed periodic inspections on procedures, and developed procedures, documented, and utilized the control of potentially hazardous energy associated with the Arpeco while employees perform blade changes. The lack of machine-specific energy control procedures exposed employees to hazardous energy during servicing operations.  h) On or about 04/08/2025, the employer failed to establish and implement an energy-control program which provided training, performed periodic inspections on procedures, and developed procedures, documented, and utilized the control of potentially hazardous energy associated with the Nilpeter Press 13 inch 8 color which the employees perform die changes, roll changes, blades changes, change the ink, grease, oil, and clean the machine without performing LOTO. The lack of machine-specific energy control procedures exposed employees to hazardous energy during servicing operations.  i) On or about 04/08/2025, the employer failed to establish and implement an energy-control program which provided training, performed periodic inspections on procedures, and developed procedures, documented, and utilized the control of potentially hazardous energy associated with the Rhyguan die-cut rewind Top 330-3R which the employees perform die changes, roll changes, blades changes, change the ink and clean the machine without performing LOTO. 29 CFR  1910.178(l)(2)(ii):Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  a) On or about April 8, 2025, the employer failed to ensure that powered industrial truck operator training consisted of the required combination of (1) formal instruction, (2) practical training, and (3) evaluation of the operator's performance in the workplace before allowing employees to operate a Clark C25CL (Serial No. C232L-0088-9823-KF). Employees were permitted to operate the forklift without having completed all required elements, exposing the operator and nearby employees to struck-by and caught-between hazards.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) On or about 04/08/2025, the employer failed to provide one or more methods of machine guarding on the Slitter Rewinder Vertaflex Racer while an employee was running a label job at the rewind and using their hands to touch/remove dust or lint from labels in motion at the point of operation, in that no guards were installed around the rewind rollers to prevent employee contact during operation, exposing employees to amputation and caught-in/entanglement hazards.  b) On or about 04/08/2025, the employer failed to provide one or more methods of machine guarding on the Slitter Rewinder Rhyguan while an employee was performing a roll changeout while labels were running at the roll/nip area, in that no guard was provided where rolls are changed out, exposing the employee to caught-in/entanglement hazards; additionally, the foot pedal was not adequately guarded (not fully enclosed), which could lead to unintentional start-up of the machine, exposing employees to sudden-motion and entanglement hazards.  c) On or about 04/08/2025, the employer failed to provide one or more methods of machine guarding on the Weldotron Model 7112 while an employee was working at the discharge end, at the conveyor, in that the conveyor at the end of the line was not adequately guarded (explain how/why), exposing employees to caught-in hazards at moving parts.
