29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). (a) Building exterior; On or about April 4, 2025, a worker in an elevated JLG model 860SJ aerial lift (Serial No. 0300184088) was not wearing a body belt or harness and was not attached to the boom or basket.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Building roof: On or about April 4, 2025, the employer did not ensure each employee performing work from the steep-slope roof of a new construction commercial building, approximately 12-feet, 6-inches above the ground, was protected from falls by using a guardrail system, safety net system, or personal fall arrest system. Maple Ridge Builders Corp was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1743149, citation number 1, item number 1 and was affirmed as a final order on May 13, 2024, with respect to a worksite located at 3619 Gateway Dr, Eau Claire, WI, 54701.
