On or about April 8, 2025, at the above addressed jobsite, an employee working on the ground, under a worker performing residential roofing operations, was not wearing a hard hat to protect against falling materials and debris.

29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about April 8, 2025, at the jobsite located at 19125 Boulder Ridge Ct. Mokena, Illinois 60448, employees were exposed to eye injury while operating a pneumatic nail gun without wearing eye protection while installing roof shingles.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about April 8, 2025, at the above addressed jobsite, employees performing residential roofing operations, fourteen (14) feet nine (9) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  b. On or about April 8, 2025, at the above addressed jobsite, employees performing residential roofing operations, fourteen (14) feet two (2) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   a. On or about April 8, 2025, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface nor was it secured at the top, thereby exposing employees to the hazards associated with falls.
