29 CFR 1926.20(b)(2): The employer did not develop an accident prevention program that provided for frequent and regular inspections of the job sites, materials, and equipment, to be made by competent persons designated by the employer: a. 808 Pavonia Ave, Hudson, NJ On or about 04/09/2025 the employer did not develop an accident prevention program for employees exposed to fall hazards of up to 24 ft. while performing demolition activities to include using fall protection improperly with a lifeline wrapped around a steel beam used as an anchor point.

29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded: Location: a) Worksite; 808 Pavonia Ave, Hudson, NJ 07306 a) On or about 4/9/2025, an employee was exposed to fall hazards of approximately 24 feet from the third floor level to the ground below while performing work such as, but not limited to, demolition activities. The employee used a lifeline, as part of a fall protection system, which was wrapped around a steel H-beam without protection for cuts/abrasions.
