29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. Jobsite: On or about April 9, 2025, the employer did not ensure that an employee removing an auger from a Leeboy 5300 paving machine was trained to ensure that a complete shut down of the machine and removal of stored energy was completed prior to removing the auger. The employee suffered a serious laceration to his right hand and a dislocated thumb when the auger rotated as he tried to remove it from the paving machine.
