29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) On or about April 9, 2025, employees working on the ground to perform cleanup and assist the roofers who were working directly above while re-roofing the structure, were not protected from struck by hazards from falling materials and tools by wearing a protective helmet.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about April 9, 2025,  employees conducting roofing activities (shingle installation) on an 8:12 pitch roof of the structure were not protected from falling to the lower level. This exposed employees to a fall hazard of 11 feet at eave, and to 17 feet at the gabled ridge end.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  (a) On or about April 9, 2025, the portable ladder(s) being used by employees to access the roof of the structure  did not extend at least three feet above the upper point of contact with the eave, nor were ladders secured in place. This exposed employees accessing and egressing the roof to a fall hazard of 11 feet onto grass, building materials, and  concrete.
