(b) On or about April 9, 2025, an employee roofing a house at 4233 Daisy Hollow Loop, Mesquite, TX. was not protected from falling while using an aluminum extension ladder placed up against the roofs edge at the front of the house with its side rails extended approximately six inches above the roofline.  (c) On or about April 9, 2025, an employee roofing a house at 4237 Daisy Hollow Loop, Mesquite, TX. was not protected from falling while using a Werner aluminum extension ladder placed up against the roofs edge to the left of the front door of the house with its side rails extended approximately two feet above the roofline.
