29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about April 9, 2025, an employee roofing a house at 4233 Daisy Hollow Loop, Mesquite, TX. was not protected from falling while using a Werner aluminum extension ladder placed up against the roofs edge on the side of the house towards the middle with its side rails extended approximately eighteen (18) inches above the roofline. (b) On or about April 9, 2025, an employee roofing a house at 4233 Daisy Hollow Loop, Mesquite, TX. was not protected from falling while using an aluminum extension ladder placed up against the roofs edge at the front of the house with its side rails extended approximately six inches above the roofline. (c) On or about April 9, 2025, an employee roofing a house at 4237 Daisy Hollow Loop, Mesquite, TX. was not protected from falling while using a Werner aluminum extension ladder placed up against the roofs edge to the left of the front door of the house with its side rails extended approximately two feet above the roofline.
