29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) Jobsite located at 1011 W. Baltimore Pike, West Grove, PA 19390 on or about April 9, 2025. The employer failed to install protective systems in an excavation. Employee was observed in an excavation, approximately 10 feet in depth, thereby exposing the employee to struck-by and crushing hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. a) Jobsite: 1011 W. Baltimore Pike, West Grove, PA 19390 on or about April 9, 2025. The employer did not ensure the employee working inside an approximate 10 foot deep excavation was provided with safe access by extending the side rails of the extension ladder inside the excavation at least 3 feet above the upper landing surface.
