29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New construction located at 3102 Marbella Circle Kennesaw, GA 30152: lots 1-4 Roof, the employer exposed employees to 21-foot fall hazards from the  5/12 pitch roof, in that fall protection systems use was not enforced to employees installing shingles.

29 CFR  1926.502(d)(1): Personal fall arrest system connectors were not drop forged, pressed or formed steel, or made of equivalent materials:  a) New construction located at 3102 Marbella Circle Kennesaw, GA 30152: lots 1-4 Roof, the employer exposed employees to 21-foot fall hazards from the  5/12 pitch roof, in that proper fall protection systems were not used in-that rock climbing harnesses were used during shingle installation activities. Additionally, the anchor point was not metal to metal as it was connected with a knot in the rope.
