29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. At an establishment located at 659 Main Street, Reedsville, Wisconsin; (a) On or about April 3, 2025 and thereafter, in the Paint Room, the Komatsu propane forklift, Model: FG30HT-12, S/N: 501483A, had a hole in the fork which was used to move trailers around the facility without the manufacturers prior approval. The modification to the fork would affect the capacity and safe operation exposing employees to struck by hazards. (b) On or about April 8, 2025, the yellow Hyster, Lift Truck Model H55XM, S/N: D177B03923P, had a hole in the fork which was used to move trailers around the facility without the manufacturers prior approval. The modification to the fork would affect the capacity and safe operation exposing employees to struck by hazards.

29 CFR 1910.178(a)(6): The user shall see that all nameplates and markings are in place and are maintained in a legible condition. On or about April 3, 2025, at an establishment located at 659 Main Street, Reedsville, Wisconsin, in the Paint Room, the Komatsu propane forklift used to move trailers around in the Paint Room did not have a nameplate in legible condition exposing employees to potential tip over hazards.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. At an establishment located at 659 Main Street, Reedsville, Wisconsin; (a) During the week of March 31, 2025 and thereafter, the employer did not conduct daily inspections of the powered industrial vehicles such as but not limited to the Toyota propane forklift, Komatsu propane forklift, and Hyster propane forklift before putting it into service to identify defects.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. During the month of March 2025 and thereafter, at an establishment located at 659 Main Street, Reedsville, Wisconsin; the Promecam RG-35-20 press did not have point of operation guarding when utilizing a foot pedal to activate the press exposing employees to caught in between and struck by hazards.
