29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about April 9, 2025, employees were exposed to struck by hazards, while performing framing work on a residential home located at 3542 Joyce Ave NW, Massillon Ohio 44646; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material using air driven nail guns.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about April 9, 2025, employees were exposed to fall hazards, while performing roofing work on a residential home located next to 3542 Joyce Ave NW, Massillon Ohio 44646, the employer did not ensure employees working on a residential roof were protected from a fall of approximately 11.2 feet by utilizing a fall protection system.  DJM Construction and Carpentry LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to use Fall Protection), which was contained in OSHA inspection number 1569645, citation number 01, item number 01 and was affirmed as a final order on (1/20/2022), with respect to a workplace located at 4528 Binfield Cir, Uniontown Ohio 44685.  DJM Construction and Carpentry LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to use Fall Protection), which was contained in OSHA inspection number 1553777, citation number 01, item number 02 and was affirmed as a final order on (1/20/2022), with respect to a workplace located at 930 Honeysuckle Dr, North Canton Ohio 44720.
