29 CFR 1910.217(c)(1)(i):The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power presses: On or about April 25, 2025, Gregory Container Company's facility located at 1385 Industrial Drive in Kahoka, Missouri, exposed workers to caught-in-between hazards while working at the point of operation of an unguarded brake press and shear machine. The two brake presses and a shear machine were in use and not properly guarded during the inspection. Contact with unprotected point of operation may result to amputation and other debilitating health condition.

29 CFR 1910.178(l)(3)(i)(J): Training did not include vehicle inspection and maintenance that the operators are required to perform: On or about April 25, 2025, Gregory Container Company's facility located at 1385 Industrial Drive in Kahoka, Missouri, did not document daily inspection logs for the forklift equipment used at their facility. The absence of inspection before operating a forklift and other powered industrial trucks may pose a vehicular hazard by not identifying or recognizing equipment defects that may result in an accident, resulting from an equipment failure, injury to personnel, or property damage.
