29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly: Auto Shop: On April 9, 2025 and at times prior thereto, the Toyota fork truck had a steel drum grabber attachment that was not approved by the manufacturer prior to use. This condition potentially exposed employees to dropped loads and crushing hazards.

29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed: CSMS Shop: On April 9, 2025 and at times prior thereto, the employer did not maintain monthly certification records for the hoist chains on the DeMac overhead crane. This condition potentially exposed employees to dropped loads and crushing hazards.
