29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a)?Production Area?- On?or about?April 11,?2025, the?employer did?not ensure?an energy?control program?consisting of?an energy?control procedure,?employee training?and periodic?inspections was?established for?employees performing?service and?maintenance on?machines including,?but not?limited to,?moulders, mixers, slicers, dividers and ovens.?The employees?were thereby?exposed to?hazards associated?with rotating?and moving?parts.

29 CFR 1910.263(e)(1)(viii): Horizontal dough mixer(s) were not equipped with a full enclosure over the bowl while the agitator was in motion:  a) Production Area - On or about April 11, 2025, employees were operating the Hobart Mixer M-802 SN 1629631 and the employer failed to safeguard the mixing bowl, thereby, exposing employees to amputation hazards.

29 CFR 1910.263(g)(1): Mechanical feed moulder(s) were not provided with hoppers so designed and connected to the proofer as to prevent employee contact with in running rolls:  a) Production Area - On or about April 11. 2025, employees were operating the Benier USA Bread Divider Machine SN 10081 and the employer failed to guard the opening of the hooper that is attached to the machine. Thereby, employees are exposed to amputation hazards.  b) Production Area - On or about April 11. 2025, employees were operating the Fortuna Moulder Baking Machine SN KY-810 and the employer failed to guard the opening of the hooper that is attached to the machine. Thereby, employees are exposed to amputation hazards.

29 CFR 1910.263(j)(1)(iii): Cover(s) over the knife head of reciprocating blade slicer(s) were not provided with an interlocking arrangement so that the machine could not operate unless the cover was in place:  a) Production Area- On or about April 11, 2025, employees were operating the Oliver Slicers Model 797G, SN 108433 and 115890 without any guards over the blades due to the employer failing to effectively guard the machines, thereby exposing employees to amputation hazards.
