29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. a. On or about April 10, 2025, at the worksite: employees were exposed to an an approximate 11'-7" fall hazard while using a 24-foot Werner fiberglass ladder to access the roof, in that the ladder side rails extended approximately 24 inches above the roof landing. b. On or about April 10, 2025, at the worksite on the second level: employees were exposed to an approximate 5 foot fall hazard while using a 6 ft A Frame Step Ladder to access the roof, in that the ladder did not extend at least 3 feet above the roof landing.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a. On or about April 10, 2025 at the worksite: employees were exposed to a 13 foot fall hazard while installing metal roofing on a flat roof in that, fall protection was not used. b. On or about April 10, 2025 at the worksite: employees were exposed to a 30 foot to 40 foot fall hazard while installing metal roofing on a 3:12 pitched roof in that fall protection was not used. The High Quality Roofing Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1) , which was contained in OSHA inspection number 1634948, citation number 1, item number 1 and was affirmed as a final order on 3/21/2023, with respect to a workplace located at 6456 Mossy Woods Orlando, Fl 32829.
