In the shop area, at 291 Springview Commerce Drive, Debary, Florida:  On or about April 8, 2025, the employer exposed employees to amputation hazards, in that procedures were not developed and utilized for the unexpected energization, or start-up of the machines or equipment,  while employees conduct maintenance operations.

In the shop area, at 291 Springview Commerce Drive, Debary, Florida:  On or about April 8, 2025, the employer exposed employees to an amputation hazard, in that the Jet Drill Press did not have a guard properly installed to provide protection of the point of operation and rotating parts.

In the shop area, at 291 Springview Commerce Drive, Debary, Florida:  On or about April 8, 2025, the employer exposed employees to struck by hazards, in that electrical wiring disconnecting the control panel of the collapsible cantilever mandrel was not identified for employees during fabrication work.
