29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about April 8, 2025, on a residential building, employees performing roofing work activities were using portable extension ladders to access the upper levels and the side rails did not extend at least three (3) feet above the landing: a.) On the west side of the building, an employee used a portable ladder where the side rails did not extend at least 3 feet above the upper landing surface. b.) On the northeast side of the building, an employee used a portable ladder where the side rails did not extend at least 3 feet above the upper landing surface.
