29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a) On or about April 9, 2025, an employee was not protected from being struck-by debris to the eyes by not wearing eye protection when using a nail gun while doing roofing work. (b) On or about April 9, 2025, an employee was not protected from being struck-by debris to the eyes by not wearing eye protection when working below those doing roofing work above.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about April 9, 2025, an employee was not protected from falling while using a Werner aluminum extension ladder placed up against the roofs edge on the side of the house towards the front with its side rails extended approximately twelve (12) inches above the roofline. (b) On or about April 9, 2025, and employee was not protected from falling while using an aluminum extension ladder placed up against the back of the house with its side rails approximately eighteen (18) inches above the roofline.
