29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about April 2, 2025 and times prior thereto employees in raw production area were exposed to machine hazards while operating a Bepex corporation Rietz blender mixer without guarding to prevent employee from making contact with the mixing blades near the output chute.

29 CFR  1910.305(b)(2)(i):All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  On or about April 9, 2025 and times prior thereto employees in the maintenance and the storage areas were exposed to electrical shocks while working in vicinity of electrical junction boxes without faceplate covers to prevent employees from making contact with up to 120V electrical conductors inside.

29 CFR  1910.305(g)(2)(iii):Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.  On or about April 9, 2025 and times prior thereto, employees in the raw processing area and storage area were exposed to electrical shock hazards while operating mixing machines in the raw production area and while working in vicinity of electrical junction boxes with strained flexible conduit energizing the dryer pump.  Instance A:  On or about April 9, 2025 and times prior thereto, employees in the raw processing area were exposed to electrical shock hazards while operating mixing machines.  The small Rietz Blender mixer used in sausage production had strained electrical connection where outer insulation was pulling away from terminal connection attached near the left size of the output of the machine .  Instance B: On or about April 9, 2025 and times prior thereto, employees in the raw processing area were exposed to electrical shock hazards while operating mixing machines.  The large Rietz Blender mixer used in sausage production had strained electrical connection where outer insulation was pulling away from terminal connection at the control box.  Instance C:   On or about April 9, 2025 and times prior thereto, employees in the storage area were exposed to electrical shock hazards while working in vicinity of electrical junction boxes with strained flexible conduit energizing the dryer pump in the storage area outside the maintenance area.
