29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about April 11, 2025, at the location of a worksite located at 27030 Hilliard Blvd., Westlake, OH: employees were exposed to a fall hazard of approximately 10 feet while performing roofing activities on a residential home . Employees were using an extension ladder to access the roof, and the employer did not ensure they extended at least 3 feet above the upper landing surface.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about April 11, 2025, the employer did not ensure employees engaged in preparing a roof for shingle installation with an 4.5/12 pitch at a residential home located at 27030 Hilliard Blvd, Westlake, OH. 10-15 feet above the ground were protected by guardrail systems, safety net system, or personal fall arrest system BEA CONSTRUCTION, L.L.C. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13) , which was contained in OSHA inspection number 1664281, citation number 1, item number 1a and was affirmed as a final order on 5/30/2023 , with respect to a workplace located at 9395 North Marks Road, Strongsville, OH 44149.
