OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential struck by and crushing hazards from over pressurized tires: a. Site - On or about April 7, 2025, an employee mounted a 16 inch tire (National Commando HTS tire 225/75R16) on a 16.5 inch rim which was an incompatible combination and began adding air to the tire. The employee left the tire unattended, and another employee began adding air to the tire without consulting the first employee or verifying the condition and compatibility of the tire and rim. The tire burst and struck the employee in the arm and face.
