(a) In the warehouse - On or about April 11, 2025, employees were exposed to entrapment hazards in that the emergency exit door between dock doors 142 and 143 was blocked by pallets of merchandise. (b) In the warehouse - On or about April 11, 2025, employees were exposed to entrapment hazards in that the emergency exit door between dock doors 156 and 157 was partially blocked by pallets of merchandise. (c) In the warehouse - On or about April 11, 2025, employees were exposed to entrapment hazards in that the emergency exit door between dock doors 163 and 164 was partially blocked by a stack of empty pallets.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. (a) In the warehouse - On or about April 11, 2025, employees were exposed to fire hazards in that the fire extinguisher at aisle P-15 was not mounted so it was readily accessible. (b) In the warehouse - On or about April 11, 2025, employees were exposed to fire hazards in that the fire extinguisher at aisle P-42 was not mounted so it was readily accessible. (c) In the warehouse - On or about April 11, 2025, employees were exposed to fire hazards in that the fire extinguisher at aisle B-01 was not mounted so it was readily accessible.
