29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:   a. On or about April 11, 2025, at the above addressed jobsite, employees performing roofing operations at eleven (11) feet two (2) inches above the ground were not protected from falls, thereby exposing employees to fall hazards.  b. On or about April 11, 2025, at the above addressed jobsite, employees performing roofing operations at nineteen (19) feet eight (8) inches above the ground were not protected from falls, thereby exposing employees to fall hazards.  Gonzalez Brothers Inc. was previously cited for a violation of an equivalent occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1758752, citation number 2, item number 1 and was affirmed as a final order on December 5, 2024, with respect to a workplace located at 3432 W. 60th Place, Chicago, IL 60629.  Gonzalez Brothers Inc. was previously cited for a violation of an equivalent occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1673910, citation number 2, item number 1 and was affirmed as a final order on August 8, 2023, with respect to a workplace located at 134 S. Oak, Westmont, IL 60559.  Gonzalez Brothers Inc. was previously cited for a violation of an equivalent occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1598014, citation number 1, item number 2 and was affirmed as a final order on September 21, 2022, with respect to a workplace located at 5631 Rosinweed Ln., Naperville, IL 60564.  Gonzalez Brothers Inc. was previously cited for a violation of an equivalent occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1547992, citation number 1, item number 1 and was affirmed as a final order on February 8, 2022, with respect to a workplace located at 1017 McCarthy Road, Lemont, IL 60439.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a. On or about April 11, 2025, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
