29 CFR 1926.20(b) Accident Prevention Responsibilities. (2):Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. a) On or about April 8, 2025, the employer failed to ensure that a competent person conducted frequent and regular inspections of the jobsite, materials, and equipment to detect and correct hazardous ladder conditions-specifically, an extension ladder set on uneven ground and positioned at an improper 4 : 1 angle. As a result, an employee fell approximately 20 feet while installing an exterior electrical receptacle and required hospitalization.

29 CFR 1926.1053(b) Use. (5)(i):Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). On or about April 8 2025, the employer failed to ensure that a non-self-supporting extension ladder was positioned at the required 4 : 1 (one-quarter working-length) angle and on stable, level footing while an employee used it to install an exterior electrical receptacle on a newly constructed residence. As a result, the employee fell approximately 20 feet and required hospitalization.

(6):Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement. a) On or about April 8 2025, the employer failed to ensure that a non-self-supporting extension ladder used to install an exterior electrical receptacle was placed on a stable, level surface or otherwise secured to prevent displacement. As a result, the ladder shifted and an employee fell approximately 20 feet and required hospitalization.
