29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) For the construction site at 6993 Jared Oaks Dr., Wesley Chapel, FL 33545, on or about 04/11/2025, the employer exposed employees to fall hazards of approximately 10 feet and 5 inches while performing framing work on a residential house, in that, the ladder used to gain access to the roof working area extended only approximately 1 foot above the accessed edge.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) For the construction site at 6993 Jared Oaks Dr., Wesley Chapel, FL 33545, on or about 04/11/2025, the employer exposed employees to fall hazards of approximately 10 feet and 5 inches while performing framing work on a residential house, in that, No fall protection was being used.  Velasco Evolution Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1729712, citation number 1, item number 4 and was affirmed as a final order on 05/17/2024, with respect to a workplace located at 11484 Brook Edge Ave Lot 37, Riverview, FL 33569.
