1910.176(b) Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. a) Dam-Bra Corporation, Steel Storage Racks throughout facility: On or about April 10, 2025 and at times prior, employees were exposed to struck by hazards while loading and unloading pallets of material including: bottled water, and Imerys Carbonates from the unsecured storage racks with forklifts. b) Dam-Bra Corporation, Steel Storage Racks throughout facility: On or about April 10, 2025 and at times prior, employees were exposed to struck by hazards while loading and unloading pallets of material including: bottled water, and Imerys Carbonates from the storage racks. The employer failed to identify the load rating of the storage racks throughout the facility to ensure they were not overloaded. c) Dam-Bra Corporation, Water Softener Tank Hoist: On or about April 10, 2025 and at times prior, employees were exposed to struck by hazards while utilizing an unrated electric hoist to move water softener tanks filled with filtering media weighing approximately 200 lbs.

29 CFR 1910.22(b): The employer did not ensure that each walking-working surface can support the maximum intended load for that surface: a) Dam-Bra Corporation, North Wall Mezzanine: On or about April 10, 2025 and at times prior, the employer failed to post load rating signs on the mezzanine located on the North wall mezzanine that contained: water softening tanks, rubber hose, and other pallets of material. b) Dam-Bra Corporation, South Wall Mezzanine: On or about April 10, 2025 and at times prior, the employer failed to post load rating signs on the mezzanine located on the South wall mezzanine that contained: plastic bottles for water, and other pallets of material.
