29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: Location: At or near the intersection of Page Avenue and Union Blvd, Saint Louis, Missouri: On or about April 7, 2025, a worker (laborer) who was not trained and qualified was allowed to drive (operate) a 2013 tandem Vibratory Compactor (Roller), Model CB64, Serial number ODJM00467. On the date above, a laborer driving the roller ran onto the asphalter pinning the operator against the the asphalter. The injuries caused laceration to the abdomen and left leg which led to amputation. AVD Language has been amended to per ISA: A laborer was not instructed in the operation of a Vibratory Compactor (Roller) Model CB64 Serial Number ODJM00467
