On or about April 14, 2025, at the above addressed jobsite, an employee providing bundles of shingles using a Tranzsporter Ladder Hoist on the ground below was not wearing a hard hat to protect against falling materials.

On or about April 14, 2025, at the above addressed jobsite, employees performing commercial roofing operations, ten (10') feet above the ground, were not protected from falls, thereby exposing the employees to fall hazards. b. On or about April 14, 2025, at the above addressed jobsite, an employee performing Commercial roofing operations, twelve (12') feet six (6") inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.
