On or about April 14, 2025, at the above addressed jobsite, an employee providing bundles of shingles using a Tranzsporter Ladder Hoist on the ground below was not wearing a hard hat to protect against falling materials.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:   a. On or about April 14, 2025, at the above addressed jobsite, employees performing commercial roofing operations, ten (10') feet above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  b. On or about April 14, 2025, at the above addressed jobsite, employees performing commercial roofing operations, twelve (12') feet six (6") inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.
