29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about April 14, 2025, at the above addressed jobsite, employees using a pneumatic nail gun did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a. On or about April 14, 2025, at the above addressed jobsite, employees performing residential roofing operations, thirteen (13) feet nine (9) inches, above the ground, were not protected from falls, thereby the exposing employees to fall hazards.  b. On or about April 14, 2025, at the above addressed jobsite, an employee performing residential roofing operations, nine (9) feet above the ground, was not protected from falls, thereby the exposing employee to fall hazards.  J ZUNO ROOFING LLC dba Javier Zuno Mendoza was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1781858, citation number 2, item number 1 and was affirmed as a final order on February 19, 2025, with respect to a workplace located at 12418 Huntleigh Rd., Homer Glen, IL 60491.  J ZUNO ROOFING LLC dba Javier Zuno Mendoza was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1740615, citation number 1, item number 1 and was affirmed as a final order on July 18, 2024, with respect to a workplace located at 17037 Oconto Ave., Tinley Park, IL 60477.
