29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a. On or about March 29, 2025, in the warehouse, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices during maintenance or servicing of the Rebar Spiral Machine #1. As a result of the inadvertent start-up of the equipment and/or the release of stored hazardous energy, an employee sustained an amputation to the left-hand pinky finger.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. In the warehouse - On or about March 29, 2025, an employee was exposed to machine hazards associated with moving parts when a machine operator sustained an amputation while clearing a jam on the Rebar Spiral Machine #1. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the service work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv)(a). a. In the warehouse - On or about April 15, 2025, the employer exposed employees to electrical shock hazards, in that flexible cords, cables and relocatable power taps were used in place of permanent wiring. An extension cord was found, connected to a relocatable power tap and daisy-chained to another extension cord, routed beneath cardboard, floor mats, and conveyor machinery, and extending across the warehouse floor to supply power to multiple appliances, including but not limited to a microwave, electric fan, and refrigerator.
