29 CFR  1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On April 11, 2025, Ridgeline Solar and Roofing LLC did not conduct an adequate inspection of the jobsite by a competent person at 5 Health Department Drive in Troy, Missouri thus exposing employees to the following hazards:  A) Multiple employees were exposed to fall hazards while roofing a commercial structure approximately 15 - 20 feet above ground level without utilizing conventional fall protection. B) The ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.
