29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On 11 April 2025, (4) four employees were observed roofing a 6/12 pitch commercial structure approximately 15 - 20 feet above ground level, at 5 Health Department Drive in Troy, Missouri, without conventional fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On 11 April 2025, an employee was observed using an aluminum extension ladder improperly while climbing on the ladder rungs at 5 Health Department Drive in Troy, Missouri, without the ladder side rails extending at least 3 feet above the upper landing surface being accessed.
