29 CFR l 910.l 78(1)(3)(i)(G): The employer failed to ensure that powered industrial truck operators received initial training in fork and attachment adaptation, operation, and use limitations: (a) Shipping and Receiving Areas - The employer's initial training content failed to provide adequate information to employees on fork and attachment adaption, operation, and use limitations in that it did not instruct operators to review capacity plates on the Vestil double Eagle Beak Fork-Mounted Drum Lift attachment Model # FMDL-2 , SIN $254219, prior to lifting to ensure they are not exceeding the lift capabilities. On or about 4/15/25. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
