29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Repackage Room: Annual audits of the lockout tagout procedures, for equipment, such as, but not limited to, Dauber repackaging machine Model CB-148, Serial Number 481529. were not conducted on or about 4/15/25. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. The (employer name) was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1718128, citation number 1, item number 3 and was affirmed as a final order on 6/26/24, with respect to a workplace located at 4000 East Goodwin Rd, Mission TX 78574.
