29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the forklift was not moved horizontally while the platform was occupied: (a)East River Contracting, LLC at 1120 Crystal Ridge Road, Brookings, SD 57006: On and about and at times prior to March 16, 2025, the employer did not ensure employees were protected from struck-by and ejection hazards in that employees were permitted to ride on platforms to aid in the transport of door panels during horizontal movement. This condition exposed employees to struck-by and ejection hazards.
