On or about April 16, 2025, at a jobsite located at 1712 North Fourth Street, in Sheboygan, Wisconsin; the employer did not enforce the use of protective helmets on the jobsite when employee(s) were exposed to struck by hazards while handing fascia and soffit material to employees on ladder and ladder jack scaffold systems.

29 CFR  1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system;   (a)  On or about April 16, 2025, at a jobsite located at 1712 North Fourth Street, in Sheboygan, Wisconsin; while encapsulating the lead along the fascia and soffit on the side of the residential home an employee was not utilizing personal fall arrest systems while working from a ladder jack scaffold exposed to fall hazards of approximately 12.3 feet.  (b)  On or about April 16, 2025, at a jobsite located at 1712 North Fourth Street, in Sheboygan, Wisconsin; while encapsulating the lead along the fascia and soffit on the front of the residential home an employee was not utilizing personal fall arrest systems while working from a ladder jack scaffold exposed to fall hazards of approximately 10.6 on the south end and 20.2 feet on the north end.

On or about April 16, 2025, at a jobsite located at 1712 North Fourth Street, in Sheboygan, Wisconsin; an employee encapsulating the lead along the fascia of a residential home were utilizing a Werner short rung ladder jack scaffold at approximately 20.2 feet exposing employees to fall hazards.

29 CFR  1926.454(b)(2):  The employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. The training shall include the correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold in question.  (a)  On or about April 16, 2025, at a jobsite located at 1712 North Fourth Street, in Sheboygan, Wisconsin; the employer did not provide required scaffold training for erectors to be able to identify hazards such as installing jack clamps to secure the planks and create a system, to not use the ladder jack scaffold over 20 feet, and proper angle to set the ladder up when used as part of the system.

29 CFR  1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  (a)  On or about April 16, 2025, at a jobsite located at 1712 North Fourth Street, in Sheboygan, Wisconsin; the red Werner extension ladder, located on the side of the residential house, used as part of a ladder jack scaffold system was positioned to an angle of 69.10 degrees exposing  an employee to slip and fall hazards.  (b)  On or about April 16, 2025, at a jobsite at 1712 North Fourth Street, in Sheboygan, Wisconsin; the silver Werner extension ladder, located on the front side of the residential house, used by an employee to install soffit and fascia material was positioned to an angle of 67.53 degrees exposing the employee to slip and fall hazards.
