29 CFR 1910.1025(d)(2): An initial determination was not made to determine if any employee may be exposed to lead at or above the action level: (a) Main Boiler Plant: On or about April 15, 2025, the employer did not perform an initial determination to determine where paint chips in the boiler room, had lead, which ranged from 0.004% to 0.33% where facilities management workers are required to perform routine maintenance duties. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.1025(l)(1)(i): Employee(s) working in an area where there is potential exposure to airborne lead at any level were not informed of the content of Appendices A and B of 29 CFR 1910.1025: a) Main Boiler Plant: On or about April 15, 2025, maintenance workers in the boiler room are exposed to flaking paint chips that contain the presence of lead that range from 0.004% to 0.33% and were not provided with training on harmful effects of exposure to airborne lead exposure. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
