The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about April 16, 2025, the employer, working at the single story residential site address of 11 Taft Street in Shelby, Ohio 44875, did not ensure their employees had been  trained on how to use appropriate fall protection systems including but not limited to the personal fall arrest system the employer owned or alternative fall protection measures. Employees of Rhodes Construction Services, LLC were removing shingles from a residential roof, without the use of fall protection, exposing employees to fall hazards of over six feet.

On or about April 16, 2025, the employer, working at the residential site address of 11 Taft Street in Shelby, Ohio 44875, did not ensure their employees were protected from fall hazards by properly using their personal fall arrest system. Employees were removing and replacing shingles from an existing roof with a 5/12 pitch on a single-story residential building without properly being anchored in or tied off to their fall protection, exposing employees to fall hazards of over six feet.
