29 CFR  1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a) Kitchen Area: On or about 3/20/25 and times prior the employees were exposed to chemical hazards in that the employer did not ensure the plumbed eye wash station was operable for employees working with corrosive hazardous material such as Systems PTS dye mixture  and Eriofast Blue 3R.

29 CFR  1910.1200(e)(1):The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) Kitchen Area:  On 4/10/25, and times prior, the employees were exposed to chemical hazards, in that a written hazard communication program was not implemented when employees are handling hazardous chemicals such as citric acid while mixing paint.   (b) Press Area: On 4/10/25, and times prior, the employees were exposed to chemical hazards, in that a written hazard communication program was not implemented when employees are handling hazardous chemicals such as isocyanates when producing polyurethane.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) Press Area: On or about 4/3/2025, the employer did not report to OSHA within 24 hours the hospitalization of an employee resulting from a work related injury. The injury occurred on 3/20/2025 and the employee was hospitalized on 3/21/25. The employer did not report the hospitalization.
